Pursuant to KRS 15.291 and 15.293 and accompanying regulations, including 40 KAR 9:010E and 40 KAR 9:020E, counties receiving opioid settlement funds must submit their quarterly certification form to the Kentucky Opioid Abatement Advisory Commission by Mar. 31, 2024.
The form, available on the Commission’s website, requires the recipient to affirm, under penalty of perjury, that its use of the funds is in compliance with KRS 15.291 and 15.293 and accompanying regulations.
In signing the form, the recipient also agrees to maintain for five years all necessary records—including, where relevant, itemized receipts, financial statements, accounting data, etc.—to authenticate the recipient’s use of its opioid funds. The Kentucky Opioid Abatement Advisory Commission reserves the right to inspect such records.
The form must be completed by an authorized individual and notarized.
Counties receiving opioid settlement funds must submit quarterly certifications to the Commission due on the following dates of the year:
- March 31
- June 30
- September 30
- December 31
